EyeCare Partners is rolling out important benefit changes in 2023 that are designed to positively impact you and your family.
Because of these important changes, this is an active enrollment. Everyone Must Re-Enroll. If you do not Re-Enroll, you will not have coverage in 2023.

The Michigan plans are slightly different due to the unique way Michigan health insurance and provider networks are structured. They have been designed to provide a range of options of very high quality at an affordable price. We are happy to be able to make this investment in you.

Important Information to Know
MEDICAL INSURANCE
EyeCare Partners partners with Anthem to offer medical insurance. We are happy to announce that most team member contributions for the medical plans will be decreasing in 2023.
For 2023, EyeCare Partners will continue to offer two high-deductible health plans (HDHPs) along with a new PPO option. In addition, you have the opportunity to open and fund a health savings account (HSA), when you enroll in one of our high deductible health plan options. For more information about these medical plans, please review the attached 2023 Employee Benefits Guide.
NEW for 2023 – Domestic partner benefits available.
DENTAL INSURANCE
EyeCare Partners will continue to partner with Delta Dental of MO to offer dental insurance.
Employee contributions for the dental plan will remain the same for 2023.
VISION INSURANCE
The EyeCare Partners vision plan will remain with VSP. Employee contributions will stay the same in 2023.
NEW – PAID MATERNITY LEAVE
We are excited to announce a new paid maternity leave benefit! Starting in 2023, we will provide 100% of pay for five weeks following birth. We’re proud to support our moms in this way!
MYQHEALTH
EyeCare Partners will continue to offer support through MyQHealth. If you have questions about your health care, ID cards, in-network providers, billing, claims or how your benefits work, the MyQHealth Care Coordinators are here to help!
HEALTH SAVINGS ACCOUNTS (HSAs) AND FLEXIBLE SPENDING ACCOUNTS (FSAs)
HSAs and FSAs will be provided through Rocky Mountain Reserve. EyeCare Partners’ matching contribution will be deposited into your HSA with Rocky Mountain Reserve.
LIFE AND DISABILITY INSURANCE
EyeCare Partners will continue to offer basic life and AD&D for you and supplemental life and AD&D insurance for you, your spouse and your children through Lincoln Financial.
Benefits and rates will remain the same for 2023.
VOLUNTARY ACCIDENTAL INJURY, CRITICAL ILLNESS INSURANCE, AND HOSPITAL INDEMNITY INSURANCE
The voluntary accidental injury, critical illness, and hospital indemnity plans will remain with Voya with no change to the benefits or cost.
